Team-based care and recovery services (also known as “case management”) provide person-centered care to individuals experiencing severe mental health and/or substance use issues.
Collaborative efforts within these services include a case manager who assesses the unique needs of each client, coordinates treatment and support services and facilitates access to community resources.
Team-based services vary by community and are adapted to individual need.
Most programs have specific eligibility criteria and require a referral to access services. Please contact your local Mental Health and Substance Use team for information on the referral and admittance process.
Assertive Community Treatment (ACT) provides a specialized team-based approach to care that is person-centered, wrap-around and recovery-oriented. ACT teams are mobile in most communities and provide individualized, community-based support for adults (19 years or older) living with complex mental health or concurrent mental health and substance use disorders. Inclusion and exclusion criteria are available through referral.

The Early Psychosis Intervention (EPI) program in Victoria provides support to individuals aged 13-35 who are experiencing their first episode of psychosis (e.g., schizophrenia, bipolar 1 or severe depression with psychotic symptoms). Services are also provided for families and caregivers of those affected.
Our multidisciplinary team works with clients to reach their wellness goals, including helping to reduce symptoms of psychosis, promote health, decrease social isolation and enhance positive coping skills.
Referrals are required and can be accessed through one of the following three channels:
Intensive Case Management (ICM) teams provide case management to individuals with persistent and severe substance use issues, with or without co-occurring mental health concerns. This voluntary service collaborates with community and healthcare partners to support individuals in improving their quality of life and independence, addressing their social relationships, physical health and mental health needs.
ICM teams consist of clinical and non-clinical members, including individuals with lived experiences. They work together to provide intensive and individualized case management and outreach services.
