Community Virtual Care provides support to people with a range of medical conditions (listed below). Our programs are suitable for those who are immune-compromised, the elderly and frail.
Registered nurses help you to manage your condition from the comfort of your home. All of the tools needed are loaned to you at no cost.
Chronic Disease Management Program
A 12-week support and education program for people diagnosed with:

A 4-week support and education program for people diagnosed with:

Pre-diabetes
Acute Illness Support
Using remote patient monitoring equipment (provided) and the phone, symptom support is provided to people with mild to moderate symptoms of the following conditions:
- Influenza
- COVID-19
- Heat-related illness
- Wildfire smoke inhalation
Virtual Supportive Care
Virtual Supportive Care is a six-month early intervention program designed to help you live well at home. Through personalized, one-on-one education and coaching with a Registered Nurse, you'll build confidence and knowledge to manage your health, respond to changes, and access additional services when needed. A comprehensive e-workbook is included to support your learning journey.
This program is not a replacement for hospitalization or doctor/nurse practitioner visits. Life threatening or significant symptoms still require you to seek medical attention from a doctor or nurse practitioner.
- What's Involved

Scheduled calls with your nurse—typically every two weeks—offer a consistent opportunity to explore topics from your client workbook, identify the supports you may need, and discuss any health goals or concerns. Together, you’ll co-develop a personalized care plan that reflects your values and priorities. Your nurse will also help communicate your goals and wishes to your broader care team, including your Primary Care Provider (GP or NP), when appropriate.
Caregiver Support Program
The Community Virtual Care Caregiver program uses virtual care technologies to support caregivers to set goals and build a personal plan that focuses on optimizing emotional wellbeing and enhancing coping skills.
The time-limited program includes three service streams:
- Brief one-to-one counselling
- Group counselling and/or
- Skill-building group

How to Register
Call your local Community Access office (numbers below) if you or a family member over the age of 19 have a diagnosis of:
- COPD, CKD, diabetes, heart failure, hypertension; or
- mild to moderate symptoms of influenza, COVID-19, heat-related illness, wildfire smoke inhalation; or
- a progressive, palliative illness; or
- you are a family caregiver
