Initial and follow-up appointments for adults and pregnant people with type 1 and type 2 diabetes, pre-diabetes, and gestational diabetes.
Our Dietitian and Nurse are trained as Diabetes Educators to help answer your questions and address your concerns.
We offer the following services:
- Blood sugar testing (glucometer provided, if needed)
- Blood glucose test strip certification
- Nutrition information / diet counselling
- Carbohydrate counting
- Review of oral Diabetes medications
- Review of injectable Diabetes medications (GLP-1 receptor agonists, insulin)
- Insulin and GLP-1 receptor agonist teaching
- Insulin adjustment
- Application of glucose monitors and education re use
- Gestational Diabetes counselling
It would be helpful to bring the following to your appointments:
- Blood glucose log book
- A three day food record
- Current list of medications (including timing and insulin dose amounts)
- Prescription for any new diabetes medication
- List of questions, concerns and/or goals
You are welcome to bring a family member or support person to the appointment with you.
Parking is limited so please come to your appointment early if you need to find a parking space.
