We offer appointments for adults, children and pregnant people with type 1 and type 2 diabetes, diabetes in pregnancy, and gestational diabetes.
Referral from a physician is not necessary, you can self-refer. A family member or friend is welcome to attend appointments with you.
Appointments are available with a Registered Nurse and Registered Dietitian.
Please consider bringing the following to your appointment:
- Blood glucose log book
- A three day food record
- Current list of medications (including timing and insulin dose amounts)
- Prescription for any new Diabetes medication
- List of questions, concerns and/or goals
Parking is limited so please come to your appointment early if you need to find a parking space.
Please note: Local pharmacies can assist with glucometer and Continuous Glucose Meter (CGM) education.
