Mount Waddington Region Diabetes Care Centre

We offer initial and follow-up appointments to adults and pregnant people living in the Woss north area including: Port McNeill, Port Hardy, Port Alice, Woss, Kyuquot, Kingcome, Guilford, Holberg, Sointula, Quatsino Band, and Fort Rupert.

Please consider bringing the following to your appointment:

• Blood glucose log book
• A three day food record
• Current list of medications (including timing and insulin dose amounts)
• Prescription for any new diabetes medication
• List of questions, concerns and/or goals

You are welcome to bring a family member or support person to the appointment with you.

Parking is limited so consider coming to your appointment early if you need to find a space.
