Geriatric Medicine Programs

Geriatric medicine programs include geriatricians, general practitioners, Care of Elderly physicians, nurse practitioners and GSS clinicians (nurses, occupational therapists, physiotherapists, and social workers). Together, they provide comprehensive health assessments, short-term treatment, and follow-up care

Geriatric medicine programs are based in Campbell River, Comox Valley, Port Alberni, Cowichan, Nanaimo and Victoria.

When to choose:

A geriatric medicine program might help you or a loved one over 65 if you have complex health problems that are hard to manage with regular care. 

Am I eligible?

Typically, anyone over 65 years of age with complex health problems that might include:

  • Multiple medical issues that affect daily life
  • Memory changes or confusion
  • Frequent falls or trouble walking
  • Unexplained weight loss or weakness
  • Mental health concerns like depression or anxiety in later life

How to access?

A doctor or nurse practitioner referral is required to access this service. 

What to expect

Once your referral is received, you will be contacted by a Community Access clinician to discuss your referral. 

Cost and fees

This service is free.
