CLINIC - Home IV Program

The Home IV Program provides assessment and ongoing education and support for those in the community undergoing long-term intravenous antibiotic therapy treatment.  This allows patients to undergo recovery in their homes in collaboration with a supportive and specialized healthcare team that includes pharmacists, nurses and physicians.

How to get a Referral 

Physician to physician referral required. Patients must meet Home IV Program criteria and be assessed by a Home IV Nurse.
