Primary care memory clinic

The Primary Care Memory clinic (PCMC) supports people who are experiencing concerns about memory. 

This short-term service provides cognitive screening, functional assessments, education, and support for caregivers. The clinic helps determine what may be causing changes, guides treatment planning, and connects individuals to additional services within Island Health when needed.

Clinics are located in Nanaimo, Cowichan, and Victoria.

When this service can help

The PCMC may be right for you or your loved one if you’ve recently noticed new or worsening memory changes and would like support in understanding them.

Am I eligible?

You are eligible if your primary concern is memory loss.

How to access this service

A doctor or nurse practitioner referral is required to access this service. 

What to expect

Once your referral is received, you will be contacted by a Community Access clinician to discuss your referral. 

Cost and fees

This service is free.
