This service may help if you:
- Need help with movement or daily activities
- Are recovering after surgery or a hospital stay
- Want to improve strength, balance, or prevent falls
- Need a home safety check or equipment to move safely
- Want support with aging, health changes, or reaching your health goals

What to expect
- A wellness mentor (an occupational therapist, nurse, or recreation therapist) will contact you.
- You will talk about what matters most to you, your health concerns, and your goals.
- Together, you’ll make a simple plan to support your health and safety at home.
- You’ll have regular check-ins (by phone or in person) to see how you’re doing and adjust your plan.
- Your mentor can share updates with your family doctor or nurse practitioner so your care is connected.
Costs and fees
The service is free, but you may have extra costs for equipment or supplies. You may need to rent or buy equipment like walkers, bath seats, or wheelchairs. In some cases, the Red Cross may lend equipment for a short time.
