Community Health Services Locations

Your local Community Health Services clinic can offer in-person appointments as well as services in your home and community such as home support, home care nursing, rehabilitation, and case management.

Please note, these are not walk-in clinics, you must be a client of Community Health Services and have a scheduled appointment in order to receive care.
