We offer individual appointments and group review classes to adults and pregnant people with type 1 and type 2 diabetes, and gestational diabetes.
It would be helpful to bring the following to your appointments:
- Blood glucose log book
- A three day food record
- Current list of medications (including timing and insulin dose amounts)
- Prescription for any new diabetes medication
- List of questions, concerns and/or goals
You are welcome to bring a family member or support person to the appointment with you.
Parking is limited so please come to your appointment early if you need to find a parking space.
