We provide services in your home and in your community. If you’re facing challenges living safely and independently at home, we can help. 

From assistance with daily tasks like bathing and dressing, to specialized nursing care, and support with physical rehabilitation, we can help reduce the need for hospital visits by providing you with proactive care and supports. 

For those unable to access specific clinics due to health concerns, our services bridge the gap, ensuring you receive care without disruption. 

Our goal is simple: to help you to remain in your own home with the appropriate resources to support your health and independence.

The Hospital at Home (HaH) program enables hospital-level care to be provided in the comfort of your own home.
Nurses help you manage your own care at home.
A Physical Therapist (PT) or Occupational Therapist (OT) will work with you to maintain or improve your physical independence and safety.
Respite care provides specialized support to clients, giving caregivers a temporary break to rest and renew their energy.
Social Workers provide counselling and services for your safety and well-being. They focus on short-term health care crisis intervention. 
RRAD provides education, clinical consultation, rehabilitation, and specialized support to adults with complex neurological health conditions.
