This service page provides in-home and community-based services to support and improve your health and quality of life to remain independent and in your own home for as long as possible.

Nurses help you manage your own care at home.
A Physical Therapist (PT) or Occupational Therapist (OT) will work with you to maintain or improve your physical independence and safety.
Respite care provides specialized support to clients, giving caregivers a temporary break to rest and renew their energy.
Social Workers provide counselling and services for your safety and well-being. They focus on short-term health care crisis intervention. 
RRAD provides education, clinical consultation, rehabilitation, and specialized support to adults with complex neurological health conditions.
